Employee 1095-C Forms

We are pleased to inform you that the 1095 forms for 2024 are now available for pickup. These forms provide important information regarding your health coverage and may be needed for your tax filings. The Paperwork Burden Reduction Act (H.R 37997) has made changes to the distribution of the 1095 Forms.

  • No More Paper Forms: Employers are no longer required to mail copies of 1095-B and 1095-C forms unless requested. If a request is made, forms must be provided by January 31 or within 30 days of the request.
  • Electronic Distribution: Employers are permitted to distribute forms electronically.

You can collect your 1095 form from the Auditor’s office during regular business hours or call to request to have the forms sent electronically. If you have any questions or require assistance, please contact Auditor’s Office.
